The application form asks for personal details, brief facts about your professional career and details of your financial circumstances so that we can assess how best to help you. Please contact us to obtain a form.

We also require two references, one of which will should be from a professional musician.

All the information you give us is strictly confidential and will not be disclosed to any other individual or organisation without your permission.
